Speaker Bio:
Luis Gonzalez-de Paz is a public health researcher focusing on home care and primary health care evaluation. He currently works as a public health and research coordinator at the Consorci d’AtenciĆ³ Primaria de Salut Barcelona Esquerra (CAPSBE), a consortium providing primary health care to 98.000 citizens in Barcelona, Spain. His work involves analyzing the effectiveness of primary care delivery, including home care. For the past five years, he has conducted and published studies on the health of informal caregivers and is currently studying how continuity of care impacts patient outcomes with EMR data.

Summary of the talk:
The Home Care Program at CAPSBE provides care to patients who cannot attend the Primary Health Care Center due to health or functional reasons. For 15 years, care has been provided by a team of registered nurses collaborating with a family physician who alternates primary care center and home visits. In 2022, the program began integrating other healthcare professionals and a permanent family physician. This session will describe the program’s characteristics, study results of the outcomes of continuity of care, and issues related to developing integrated care.
