“Paradigm Shift: My Journey in Implementing Adaptive Clinical Trials in Academic Medicine” by Dr. Clement Ma, Centre for Addiction and Mental Health
Abstract: Clinical trials that use traditional study designs are facing high development costs and high rates of failure. Adaptive designs the use observed data in the trial to modify the trial’s conduct can improve the efficiency and accuracy of therapeutic development. Systematic reviews have demonstrated that only a small minority of trials use adaptive designs. There is a clear unmet need to implement more adaptive trials to accelerate the drug development process.
In this presentation, I share my experience in developing and implementing adaptive dose finding designs in pediatric oncology and umbrella designs in dementia prevention trials. First, I identified unmet methodological needs in upcoming trials with my clinical collaborators. Second, I developed novel methods to address the unmet need and evaluate the design’s operating characteristics using trial simulations. Third, I identified key clinical collaborators willing to implement the novel designs. Fourth, I resolved practical and logistical challenges to operationalize the design by developing standard operating procedures. Finally, I developed web-based R Shiny applications to help other investigators design and conduct adaptive trials.
